Position Summary:

The Community Health Care Planner plays a key role in supporting the delivery of accessible, culturally sensitive, and patient-centered care at Better Health Clinic. With a strong emphasis on geriatric services and preventive health, this position supports clinic operations, care coordination, and public health initiatives. This role involves working closely with clinic leadership, healthcare providers, and community partners to ensure effective care planning, particularly for older adults and underserved populations, including Arabic-speaking patients. The ideal candidate will be highly organized, culturally aware, and committed to improving health outcomes through coordinated, compassionate care.

Key Responsibilities:

  • Assist in coordinating services and preventive healthcare initiatives for geriatric patients, including scheduling, follow-ups, care plan logistics, immunization drives, chronic disease screenings, and memory and cognitive health assessments
  • Facilitate communication and care coordination for Arabic-speaking patients, including medical interpretation support
  • Liaise with community organizations and health networks to coordinate referrals to home care, occupational therapy, mental health services, and other allied health or community support providers
  • Plan and implement public health campaigns focused on education and awareness, particularly for underserved communities such as newcomers, seniors, and caregivers; covering topics like navigating the healthcare system, chronic disease management, fall prevention, mental health, and medication management.
  • Maintain and update patient education materials, ensuring accessibility and cultural relevance.
  • Provide orientation and training support for new staff or volunteers involved in patient navigation or community outreach.
  • Uphold patient confidentiality and compliance with clinic policies and healthcare regulations
  • Support the Clinic Manager in overseeing the daily administrative operations of the clinic as needed
  • Provide administrative support in managing clinic supplies, scheduling staff meetings, and maintaining compliance documentation
  • Ensure culturally sensitive communication and care for diverse populations
  • Assist with data entry, reporting, and document preparation related to patient care and clinic administration
  • Contribute to the development and implementation of administrative processes that support medical workflows and patient care
  • Track patient satisfaction metrics and help develop strategies to address care gaps and enhance the overall patient experience

 

Qualifications:

  • A bachelor’s degree in a relevant health discipline is required, along with a strong foundation in medical terminology and clinical language to ensure effective communication in a healthcare setting.
  • A minimum of 5 years of experience in a patient care setting, with at least 3 years in geriatric care.
  • A minimum of 3 years of experience in health program planning and implementation, including designing, coordinating, and evaluating preventive care initiatives, public health campaigns, and community outreach programs.
  • Strong interpersonal and communication skills to build rapport with patients, staff, and external service providers.
  • Certification or training in geriatric care or a related field is a plus.
  • Experience in planning, coordinating, and evaluating public health programs or initiatives.
  • Excellent written and verbal communication skills, including the ability to facilitate discussions and provide clear instructions.
  • Fluency in Arabic is an asset for effectively serving our large Arabic-speaking patient population.
  • Strong organizational and time-management skills, with the ability to manage multiple tasks and priorities.
  • Working knowledge of computer software programs, i.e. word processing, spreadsheets, databases, and email required.
  • Culturally sensitive and respectful of diverse patient populations
  • Ability to work both independently and collaboratively as part of a team in a fast-paced environment.
  • Ability to handle sensitive information with confidentiality and professionalism.
  • Excellent organizational and problem-solving skills with the ability to handle multiple tasks simultaneously.
  • Demonstrated professional judgment and initiative.
  • Familiarity with community health systems and auxiliary services in Manitoba is preferred.

Reporting Structure:

The Community Health Care Planner will report directly to the Clinic Manager and work closely with physicians, clinic staff, and allied health professionals.
